The faculty or staff member will be held responsible for all items checked out to his/her account by their authorized user. Faculty and staff members utilizing this services are urged to choose those people who will have access carefully and to report lost or stolen Authorized User Cards to the Library as soon as possible.

How to get an Authorized User Card:

  • Fill out an Authorized User Card Application (PDF) and submit this form to the Circulation Department in person or by mail, fax, or email (address information). This application form requires the free Adobe Acrobat Reader.
  • Circulation staff will make a card for the designee and deliver the card to the faculty or staff member's office.
  • The faculty or staff member signs the card and delivers it to the designee.
